Cagrilintide is a novel, long-acting acylated amylin analogue developed to mimic and extend the physiological actions of the pancreatic hormone amylin. By acting as an agonist at amylin receptors (AMY1, AMY2, and AMY3), as well as calcitonin gene-related peptide (CGRP) receptors, cagrilintide modulates central satiety pathways, slows gastric emptying, and suppresses glucagon secretion in a glucose-dependent manner. These combined mechanisms make it a compelling candidate for research into energy homeostasis, bodyweight regulation, and glycaemic control.
In preclinical and clinical research settings, cagrilintide has demonstrated sustained receptor engagement due to its C18 fatty diacid acylation, which facilitates albumin binding and prolongs its circulating half-life to approximately seven days — a significant advance over native amylin analogues. It has been investigated in combination with GLP-1 receptor agonists, most notably semaglutide, under the investigational combination known as CagriSema, where synergistic effects on weight reduction have been observed.
