SS-31, also known as Elamipretide or MTP-131, is a synthetic aromatic-cationic tetrapeptide with the sequence D-Arg-2′6′-Dmt-Lys-Phe-NH₂. It is specifically designed to localise to the inner mitochondrial membrane, where it interacts with cardiolipin — a phospholipid essential for maintaining mitochondrial cristae architecture and electron transport chain (ETC) efficiency.
Through its cardiolipin-stabilising action, SS-31 has been shown in preclinical research to reduce mitochondrial reactive oxygen species (ROS) production, restore ATP synthesis, and attenuate cellular apoptosis under conditions of oxidative stress or ischaemic injury. These properties have made SS-31 a subject of considerable interest across a wide range of research disciplines.
Key areas of investigation include cardiac ischaemia-reperfusion injury, age-related mitochondrial dysfunction, neurodegenerative disease models, skeletal muscle atrophy, and renal protection. Clinical-stage trials have explored its potential in heart failure with preserved ejection fraction (HFpEF) and Barth syndrome.
